Getting started with treatment at Sullivan Center
Physician referral
Beginning a physical therapy program at Sullivan Center can be as simple as talking to your primary care physician about any conditions you have that may be causing pain or restricting your activities.
Your physician may provide a referral for physical therapy, after which you can contact Sullivan Center at (423) 224-5751 to schedule an appointment.
Arriving at your appointment
Please plan to arrive at Sullivan Center 15 minutes before your scheduled appointment to complete the necessary paperwork for your first visit. Also, be sure to bring your physician order for therapy, as well as your insurance card and photo identification.
Patients younger than 18 years old must be accompanied by a parent or guardian during their first visit.
What to wear
You are encouraged to wear loose, comfortable clothing that won’t restrict movement.
What to expect
A Sullivan Center therapist will review and discuss your medical history with you. The therapist will also perform a musculoskeletal evaluation to assist in designing a specialized treatment program.
Starting treatment
Many patients begin treatment on the same day as their initial visit and evaluation.
